Registration Rate: $350 USD

*Delegates who do not wish to attend the Welcome Reception will benefit from a reduced registration fee.

Registration payment can be made by credit card (Visa, MasterCard, American Express, Diners Club or Discover) or by wire transfer. Registrant is responsible for all fees associated with a wire transfer. Please send a confirmation of wire transfer by e-mail to or by fax at +1 514 380 5383. Please include at least one of the following with your bank transfer:

  • The name of the registered delegate
  • Delegate email address
  • Registration ID

Cancellation Policy

Please send cancellations by e-mail to or by fax at +1 514 380 5383. Cancellations will be processed according to the following schedule:
By 31 July 2013: Full refund less $80 USD administrative charge
1 August – 31 October 2013: 50% refund
After 31 October 2013: No refund      

On-site Registration

On-site registration will begin on 12 December 2013 at the Westin Michigan Avenue - Chicago.

On-site Registration Hours:
12 December 2013: 15:00-19:00
13 December 2013: 07:00-18:00
14 December 2013: 07:00-17:00

Daily Registration Rate

Delegates who wish to attend the Symposium for only 1 day may register using the Daily Registration Rate of $200 USD.  Daily Registration Rates can only be purchased during the On-Site Registration Hours.

Welcome Reception Tickets

Welcome Reception tickets for guests of delegates may be purchased for $30 USD during the On-Site Registration Hours.

Substitution Policy

No substitutions are allowed. Individuals will have to cancel the original registration and the new person will have to create a new registration. Within groups, however, you will be able to substitute at any time up until the regular registration deadline (31 October 2013). After this date, you will be charged an $80 USD administrative charge per substitution/name change.
